Our Homeless to Housing (H2H) Program, funded by a collaborative effort with the Housing Collective and the Department of Mental Health and Addiction Services (DHMAS), aims to help individuals experiencing unsheltered homelessness find and maintain stable housing, while connecting them to resources and supports, including housing, clinical, medical, social, educational, rehabilitative, vocational, employment, and other services essential to achieving the goal established by the clients as defined in their Housing Services and sustainability Plan. Referrals are provided by Litchield County Outreach, Coordinated Access Network (CAN), and local providers of homeless clients.
